Mounjaro Dosing Guide: Titration Schedule & All 6 KwikPen Strengths
Everything you need to know about Mounjaro (tirzepatide) dosing, from the standard titration schedule to detailed click counts for each of the six KwikPen strengths.
How many clicks are in one Mounjaro dose?
Every Mounjaro KwikPen dose is exactly 60 clicks, regardless of the pen strength. Each pen contains 4 doses (240 total clicks). The mg per click varies by pen strength. For example, a 10mg pen delivers 0.1667 mg per click.
What Is Mounjaro?
Mounjaro (tirzepatide) is a dual GIP and GLP-1 receptor agonist manufactured by Eli Lilly. It is FDA-approved for the treatment of type 2 diabetes and is widely used off-label for weight management.
It is administered as a once-weekly subcutaneous injection using the KwikPen, a disposable prefilled pen device. Each KwikPen contains 4 doses and uses a click-based dosing mechanism with 60 clicks per full dose.
Standard Titration Schedule
Mounjaro treatment starts at the lowest dose and gradually increases every 4 weeks. Your healthcare provider will determine the appropriate titration schedule based on your response and tolerability.
| Phase | Dose | Duration | Notes |
|---|---|---|---|
| Starting | 2.5 mg | 4 weeks | Initial dose to assess tolerability |
| Titration 1 | 5 mg | 4 weeks | First maintenance-level dose |
| Titration 2 | 7.5 mg | 4 weeks | Increase if additional effect needed |
| Titration 3 | 10 mg | 4 weeks | Increase if additional effect needed |
| Titration 4 | 12.5 mg | 4 weeks | Increase if additional effect needed |
| Maximum | 15 mg | Ongoing | Maximum recommended dose |

Dose Splitting with KwikPens
Dose splitting refers to using a higher-strength pen to take smaller, custom doses, getting more injections per pen. This is a common practice for patients titrating slowly or adjusting their dose between standard strengths.
Since every KwikPen delivers a precise amount per click, you can calculate exactly how many clicks you need for any custom dose amount.
Can you split Mounjaro KwikPen doses?
Yes. Because the KwikPen uses a click-based mechanism with a precise mg-per-click value, you can dial to any number of clicks for a custom dose. For example, a 10mg pen delivers 0.1667 mg per click, so 15 clicks = 2.5mg. This gives you 16 doses instead of the standard 4.
Common Dose Splitting Examples
- • 10mg pen at 2.5mg doses → 15 clicks per dose → 16 doses per pen
- • 7.5mg pen at 3.75mg doses → 30 clicks per dose → 8 doses per pen
- • 15mg pen at 5mg doses → 20 clicks per dose → 12 doses per pen
Split Dosing: Injecting Twice Per Week
Split dosing means dividing your prescribed weekly Mounjaro (tirzepatide) dose into two smaller injections given on separate days. For example, instead of injecting 5mg once per week, you would inject 2.5mg twice per week. The total weekly dose stays exactly the same.
Some patients find that the medication's effects wear off toward the end of the week, causing hunger and cravings to return 1-2 days before their next injection. By splitting the dose into two injections, the medication maintains more stable blood levels throughout the week, which can reduce these end-of-week symptoms.
What is split dosing for Mounjaro?
Split dosing for Mounjaro (tirzepatide) means taking your weekly dose as two smaller injections instead of one. For example, 5mg/week becomes 2.5mg injected twice per week (e.g., Monday and Thursday). This does not change the total weekly dose. It is used to maintain more stable blood levels and reduce end-of-week hunger. Split dosing should only be done under medical supervision.
Who Might Benefit from Split Dosing
- • Patients who experience hunger or cravings returning 1-2 days before their next weekly injection
- • Patients who experience stronger side effects (nausea, fatigue) right after injection, which may be reduced with smaller individual doses
- • Patients on higher doses (10mg, 12.5mg, 15mg) where the medication may metabolize faster
- • Patients whose doctor has specifically recommended a twice-weekly injection schedule
Split Dosing Schedule by Dose Tier
| Prescribed Weekly Dose | Split Dose (2x/week) | Example Schedule |
|---|---|---|
| 2.5 mg | 1.25 mg | e.g., Monday & Thursday |
| 5 mg | 2.5 mg | e.g., Monday & Thursday |
| 7.5 mg | 3.75 mg | e.g., Monday & Thursday |
| 10 mg | 5 mg | e.g., Monday & Thursday |
| 12.5 mg | 6.25 mg | e.g., Monday & Thursday |
| 15 mg | 7.5 mg | e.g., Monday & Thursday |
How to Split Dose with the KwikPen
Because the KwikPen uses a click-based mechanism, you can dial any custom dose. Use our dose calculator to find the exact number of clicks for your split dose. For example, splitting a 10mg weekly dose means injecting 5mg (30 clicks on a 10mg pen) twice per week.

The Golden Dose (Overfill)
Mounjaro KwikPens are manufactured with approximately 12% more medication than the labeled amount. This overfill is a standard pharmaceutical practice to ensure patients can reliably withdraw all 4 labeled doses even with minor losses during needle priming and injection.
After using all 4 standard doses (240 clicks), some medication typically remains in the pen. This leftover is commonly referred to as the "golden dose" or "bonus dose" in patient communities.
For example, a 10mg pen contains approximately 44.8mg total instead of 40mg. After 4 standard doses, roughly 4.8mg remains, enough for about 29 additional clicks.
Estimated overfill by pen strength (~12%)
| Pen Strength | Total mg | Est. Overfill | Est. Clicks |
|---|---|---|---|
| 2.5 mg | 10 mg | ~1.2 mg | ~29 |
| 5 mg | 20 mg | ~2.4 mg | ~29 |
| 7.5 mg | 30 mg | ~3.6 mg | ~29 |
| 10 mg | 40 mg | ~4.8 mg | ~29 |
| 12.5 mg | 50 mg | ~6.0 mg | ~29 |
| 15 mg | 60 mg | ~7.2 mg | ~29 |
The golden dose is not recommended or guaranteed by the manufacturer. The overfill amount can vary between pens, and using it means relying on medication beyond the labeled content. Always consult your healthcare provider before attempting to use overfill medication.
How to Use the KwikPen
Prepare the pen
Remove the pen from the refrigerator 30 minutes before use. Check the expiration date and inspect the solution. It should be clear and colorless.

Insert the needle into your skin at the injection site (abdomen, thigh, or upper arm). Press the injection button and hold for 10 seconds. Remove and dispose of the needle safely.
Frequently Asked Questions
How long does one Mounjaro pen last?
Can I use a Mounjaro pen after 28 days?
According to the manufacturer, Mounjaro KwikPens should be used within 21 days at room temperature, or 28 days in the refrigerator. Unopened pens have a shelf life of 24 months when properly refrigerated.
However, some healthcare providers report that Mounjaro can remain effective for several months even after opening. In many countries, physicians have prescribing freedom and may recommend usage timelines beyond official guidelines based on their clinical assessment. The prescribing doctor takes responsibility for that medical judgment.
The primary reason for the official time limit is the risk of bacteria or other microorganisms entering the pen through the needle attachment point. Injectable medications are more sensitive to contamination than other forms such as tablets or capsules. Keeping the pen and injection area clean is essential, and this hygiene concern is usually the main reason behind the shorter recommended shelf life after opening.
Important: Never freeze Mounjaro, as freezing destroys the medication.
